Pre-Operative Evaluation
Many surgeons will request a pre-operative medical evaluation or "clearance" letter before scheduling an elective surgical procedure. This evaluation helps ensure that you are healthy enough to safely undergo the procedure. Dr. Berens is a board-certified family physician and can perform this evaluation (including EKG) in her office.
This appointment includes a review of your health history and medications, a physical exam, review of bloodwork, and an EKG if needed. This visit will not address any symptoms or medical concerns unrelated to your pre-operative evaluation.
Please bring a copy of your most recent bloodwork to the appointment. If you have not had bloodwork in the last 6 months, bloodwork will be ordered at the time of your visit and will be included in the cost of the visit.
This appointment is available in-office only and cannot be completed virtually.
Our office does not contract with any insurance companies or bill insurance companies for any services. A Superbill will be provided upon request that can be submitted for insurance reimbursement.
No-shows or cancellations with less than 24 hours notice (during business hours) will be charged the full cost of the visit. Please call or text us as soon as possible if you need to cancel or reschedule your appointment.
